New Student Forms

Welcome to Anna Maria College!

Hello and welcome to the AMC Community! We've been preparing for your arrival and look forward to having you on campus soon. Our summer and fall orientation programming is designed to introduce you to our diverse academic programs, traditions, campus pride and community while preparing you for success during your transition to college. We hope your orientation experience provides you the opportunity to create meaningful connections with your peers and our community that will form the foundation of your success at AMC and beyond. Our orientation team is ready and waiting to welcome you to our campus, but before we do there are some things to take care of first!

On the left hand side of the page are links to a number of forms. Many of these forms can be filled out online though some must be printed and returned to the college. Should you have trouble accessing these forms please contact the Anna Maria College Orientation Office at (508) 849-3304 or through email.
 
Noel Levitz Self-Assessment
All incoming students must complete the Noel Levitz College Student Inventory. This survey takes about 30 minutes to complete. The results are used for advising purposes and are made available to students throughout the advising process. This survey provides you and your advisor with critical information about your motivation, learning styles and other strengths that will lead to college success. All students are required to complete the Noel Levitz Inventory prior to course registration. There will be time allotted to the Noel Levitz during all summer SOAR dates and fall orientation activities.

Health Forms
Establish your medical file with our Office of Health Services and confirm that you have all state required immunizations and physicals. Submitting your medical file with the Office of Health Services DOES NOT waive the student health insurance from your tuition bill.  For further information on waiving the health insurance, please visit the Student Account’s Office webpage

Student Authorization (FERPA)
Allows Student Accounts and Financial Aid to talk to your parents/guardians or other representatives about your bill and tell us what to do if there’s a credit balance on your account each semester. THIS FORM ONLY PERTAINS TO FINANCIAL INFORMATION. IT DOES NOT ALLOW THE COLLEGE TO SHARE STUDENTS' ACADEMIC INFORMATION WITH PARENTS OR GUARDIANS. THE ACADEMIC FERPA WAIVER FORM IS AVAILABLE THROUGH THE AMC REGISTRAR'S OFFICE.
 
Student Agreement Form
In exchange for the opportunity to enroll at Anna Maria College, to receive education services, and to receive other goods and valuable consideration, all incoming students are required to complete the Student Agreement Form.
 
Student Title IV Authorization (Credit Balance)
As a recipient of Federal Student Aid (also called Title IV funds) you have options regarding how some of the aid is applied to your student account.  Anna Maria College needs to know your responses to the following in order to handle your funds in the manner you prefer.
 
eREFUND
Allows you to receive your funds faster and deposited directly into your bank account if you have an overpayment on your student account. Once your account has been analyzed and approved, Anna Maria College will deposit the funds directly into your bank account upon overpayment and/or disbursement of all financial aid funds each semester.

eRefund Instructions
  1. Go to Online Resources
  2. Sign on with your username (student ID) and password
  3. Select "eRefund Sign Up"
  4. Enroll in eRefund in the bottom left corner of the webpage

Press Release
When you make Dean’s List or achieve other accomplishments, we will notify your local paper, but only if you give us permission. This form will grant that permission.
 
Housing Agreement
If you want to live on campus, you must complete this to let us know about yourself. Room assignments are not made until these forms are submitted, so don’t delay.
 
Athletics Forms
These are the forms required by any student who wishes to compete on one of our intercollegiate athletic teams.
